eForms Flags
Use this page to enable Signatures for eForms, enable Division Security, enable New York Auto ID Cards, and personalize Driver names.
How to access this page
From the Sagitta Home page:

Click Other.

Click Personalization > System > eForms Flags. The eForms Flags page appears.
Fields
Signatures – allows a signature to be selected when creating a new eForm. The signature is created in Staff Codes:

On – a list of signatures is available when printing an eForm.

Off – a list of signatures will NOT be available when printing an eForm.
Signatures Division Security – allows a user to enable the division security feature in Staff Codes. Division Security only applies when the Signatures option is "on."

On – allows a user to limit the signature selection. When you create a new eForm, the signature selection box will filter the signatures that appear by division. The system will compare the STAFF DIVISION of the user to the divisions associated to each signature, and it will only display those that match or that have been identified as selectable by all divisions.
If no signatures match the STAFF DIVISION, the system will not display the signature selection field when generating an eForm.

Off – Sagitta will not filter the signature selections, and any signature may be selected.
Driver Last Name First – setting this flag will cause driver names on policies with Last Name entered first to be constructed in the correct order on Vertafore eForms

Personal – drivers on personal lines policies will print last name first on eForms

Commercial – drivers on commercial lines policies will print last name first on eForms

Both – drivers on both personal and commercial lines policies will print last name first on eForms

For New York Auto IDs: If driver names are entered last name first, and the Last Name First flag is set to On, selecting a Driver type name in the Overrides section of the Auto ID page will result in a name formatted incorrectly. Retype the name to enter the data in the correct fields. |
Extend Expiration Date on Auto IDs – allows a user to override the Expiration Date on the Auto ID Card Creation page to issue Auto ID cards with future expiration dates.

Yes – the Expiration Date can be overridden (date cannot be prior to the Effective Date.)

No – the Expiration Date cannot be overridden.
New York Auto ID Cards Section
NY Auto IDs – enables the user to issue New York Auto IDs cards:

Off - when the flag is set to blank or Off, all fields in the section are accessible, and none is required. If one of the New York Auto ID cards (FH-1, FH-1B, FS-20, FS-21, FS-83) is selected for a vehicle registered in New York when creating Auto ID Cards on a policy, while the feature is set to Off, the ACORD 50 (non-state specific) card will be generated instead.

On - when the flag is set to On, some of the fields are required or not accessible, as outlined below.
Global? – allows New York Auto ID cards to personalize issuer license numbers and issuer keys by division:

Yes - the Issuer License Number and Issuer Key entered on this page will be used for all NY Auto IDs.

No – the field, Maintain NY Issuer Keys, displays and the Issuer License number and Issuer Key fields are not displayed. NY Auto IDs will use Issuer License Numbers and Issuer Keys by division.
Maintain NY Issuer Keys – a search button will allow the user to view and add NY.AUTOID.LICENSES to personalize by division.

Issuer License Number, Issuer Key - these fields contain licensing information issued to you by the New York Department of Motor Vehicles when you registered with them to create Auto ID Cards. The Issuer Key will become masked once the page is saved. When data is entered into one of these fields, the other two fields become required.
Issuer Name/Address - the values in this field are Global, Division, and Department:

Global - issuer name and address on New York Auto ID cards will be taken from the name and address fields below this field.

Division - issuer name and address will be taken from Division Master Maintenance for the division on the policy.

Department - issuer name address will be taken from Division Master Maintenance for the department on the policy.
When Division or Department is selected, the Agency/Issuer and the address fields in eForms Flags will be grayed out and become inaccessible; however, if data has been entered in these fields, it will be retained.
Agency/Issuer, Address Line 1, Address Line 2, Address Line 3 (Optional), Additional Text (I.E Phone/Fax#) - when Global is selected in Issuer Name/Address, the data entered in these fields will go to the Issuer field on New York Auto ID cards.
