In AMS360, information flows from the Policy > Prior Carrier fields into this section on Page 2 of ACORD 130.
Prior Carrier and Loss History data for the policy applicant appear together on the form. Prior Carrier information (in this example, from Dave's Insurance Co.) appears in sequence following Loss History information (in this case, from ABC Insurance).
