ACORD 137 NY Form 2023/08: New York Commercial Auto

We have added the most up-to-date ACORD 137 NY form (2023/08) and removed the outdated ACORD 137 NY form (2014/12).

Within the Customer Center: Customer > Policy > Basic Policy page > Line of Business (New or Edit):

• LOB: Business Auto, Application: New York Commercial Auto, 137, and Application Version 08/2023 OR LOB: Truckers, Application: New York Commercial Auto, 137, and Application Version 08/2023.

• Once you have clicked on the blue hyperlinked LOB title, "Business Auto" or "LOB: Truckers," please be sure to complete all fields that are relevant to your Customer/Policy.

  • Once you have saved all the necessary information, click on either the "eForms" button or on "Operations - eForms manager":

    • The E-Forms page will open. Select "eForms- Additional Forms (Integrated). Choose from the grid either "Business Auto" or "Truckers" and confirm it is the correct version. Click "Create."

    • The ACORD 137 will then be created:

      • If you have Business Auto, those fields will show mapped information from your Policy. The Trucker and Motor Carrier sections will be blank.

      • If you have Trucker and Motor Carrier sections, those fields will show mapped information from your Policy. The Business Auto section will be blank
