ACORD 212 Form: Commercial Umbrella - Overflow Underlying Schedule

Previously, when a user had an overflow from ACORD Form 13: Commercial Umbrella, both the 2016 and 2017 versions was incorrectly mapped. (The information is mapped to Underlying Liability Policies overflow (OFCUAUTO), Comml Umbrella - Addtl Underlying General Liab (General Liability), and Comml Umbrella - Addtl Underlying Employers Liab (Employer Liability)

Now, the aforementioned overflow pages have been replaced with ACORD 212, which will automatically attach to ACORD 131, while the information connected to the "Other Liability" fields in AMS360 will be mapped into the ACORD 212 form.

Within the Customer Center: Customer > Policy > Basic Policy page > Line of Business (New or Edit):

  • Umbrella (C) for LOB - Application: Umbrella Section, 131- Version: Can be either the 2016 or 2017 one. Click "Save."

    • Once you have clicked on the blue hyperlinked LOB title, "Umbrella (C)," please be sure to complete all fields that are relevant to your Customer/Policy.

    • Once you have saved all the necessary information, click on either the "eForms" button or on "Operations - eForms manager":

      • The E-Forms page will open. Select eForms - Additional Forms (Integrated). Choose from the grid Umbrella (C) Umbrella Section. Click "Create."

      • The ACORD 131 will then be created:

        • If you have included information that filled in the two other liability underlying insurance sections on ACORD 131, the ACORD 212 will be attached automatically . You will see your information mapped into the ACORD 212 form.
