Within the Customer Center: Customer > Policy> Basic Policy page > Line of Business (New or Edit):

• Private Passenger Auto for LOB - Application: New York Personal Insurance 88 - Version: 2022/02 >Save

  • Once you have clicked on the blue hyperlinked LOB title, "Private Passenger Auto," please be sure to complete all fields that are relevant to your Customer/Policy.

  • Once you have saved all the necessary information, click on either the "eForms" button or on "Operations- eForms manager":

    • The eForms page will open. Select eForms > Additional Forms (Non-Integrated). Under the ‘Number’ field, select 88. Choose "New York Personal Insurance Application" from the grid and then click "Create."

    • Once this has been created, you will see that certain fields are mapped to the ACORD form, while others are not.
