Forms Activity

Use Forms Activity to attach a point-in-time copy of a form to an Activity. When attached to an activity, the form is a permanent, unchangeable copy of the form as it appears at that moment.

ClosedAccessing Forms Activity
  1. On the 360 Toolbox menu, click eForms.
  2. Search for and select the appropriate customer.
  1. When eForms Manager opens, select the policy and transaction effective date from the Customer/Policy Transaction/Form Tree. Click Refresh if needed.
  2. From the eForms Manager menu, select Operation > Forms Activity. The Form Activity Logging selection form appears in the right pane of the window.

The following menus are available in a data entry form. Click each menu item to see detailed information. Note that the available menus vary depending on the form.

ClosedLogging a Form Activity
  1. From the Customer/Policy Transaction/Form Tree in eForms Manager, select the folder or form you want to attach to an activity. Then, from the Operation menu, select Forms Activity. The Form Activity Logging selection form appears in the right pane of the window.
  2. Make your form selections for the activity by clicking Add Form Based on the following Selection Options. This adds the selected forms to the grid above. Then, add an Activity Description. See Form Activity Logging Selections in this topic for specific field information.
  3. When you have made your selections, click Create in the upper right corner of the Form Activity Logging selection form. An activity is created with the selected form(s) attached.
  4. The icon beside the logged form in the Customer/Policy Transaction/Form Tree appears with blue shading, indicating it has been attached to an activity.

You can still edit or update a copy of the form in eForms Manager. However, the version of the form attached to the activity cannot be edited.

Action

Select an Action to categorize the activity. The default Action is ACORD Forms, but can be changed.

Claim

Use to identify the claim to which this activity applies. If you selected a loss notice from the Customer/Policy Transaction/Form Tree then the claim information fills for you.

Add Forms based on the following Selection Options

Select one or more of the following options to add forms to the grid for attachment to the activity:

Selected Transaction

Pulls all of the forms in the highlighted transaction within the Customer/Policy Transaction/Form Tree. The forms appear in the grid, in the order they are listed in the tree.

Clicking Delete, you can select individual or multiple forms in the grid and delete them.

Forms with multiple pages are listed in the grid multiple times representing each page.

Selected Folder

Pulls all forms in the highlighted folder in the Customer/Policy Transaction/Form Tree. The forms appear in the order they are listed in the tree.

Clicking Delete, you can select individual or multiple forms in the grid and delete them.

Forms with multiple pages are listed in the grid multiple times representing each page.

Selected Form

Pulls only the highlighted form in the Customer/Policy Transaction/Form Tree.

If you select individual forms and add them one at a time to the grid, they are listed in the order you add them.

Changed since last Activity

Pulls all forms that have not been attached to an activity, or that have been changed since attached to an activity.

Activity Description

Enter a description for the activity.
