Report: Certificate Holders

This feature, or some fields and options described, might not be available depending on your settings, security rights, or platform package.

The Certificate Holder report is a listing of all individuals and companies (names and addresses), for whom certificates were created. You can run this report at renewal so an insured can review the list of certificate holders from the last policy term. The customer can then indicate any changes or additions needed for the renewal term.

Access this report from any center or form that has a Toolbox menu. Selecting Toolbox > Reports takes you to the Reports - All list.

Click Certificate Holders. The Reports - Certificate Holders selection form opens. Make your selections, and then from the File menu, select one of the following:

Print
Print Preview
Create CSV to export the report to the program of your choice.

Customer/Certificate Selection

Choose Certificate Holders for a specific customer.

Option/Field

What is this?

Customer

To locate the customer, enter one or more characters of the customer name and then click Search. The Customer Search form opens for you to make your selection.

Certificate

The list includes all Certificates for the selected customer. Select All (default) or a specific Certificate.

Holder Selection

Options	What is this?
All Holders	Select to see all holders entered for the selected Certificate(s).
Holders added in the last ___ year(s)	Select and enter a number, to see all holders entered for the selected number of years, for the selected certificates.
Select Specific Issue Dates	Select to see all Certificate holders for the From/To issue dates entered, for the selected certificates.

Report Options

Group/Field	What is this?
Print Cover Page	Allows you to include a cover page listing the selected options included in the report. This option defaults as unchecked.
Description	Accept the default description, or enter a new one that describes the report you are requesting.

Report Fields	Description
Customer Name	The Customer name. If both a firm name and a last name exist, the firm name appears first, followed by the DBA name, and then the last name, first name.
Certificate Number/ Description	The certificate number and description entered when the certificate was created.
Certificate Holder	The certificate holder's name.
Contact	The certificate holder's contact.
Address	The certificate holder's address.
City State Zip	The certificate holder's city, state, and zip.
Fax Number	The certificate holder's fax number.
Email	The certificate holder's email address.
Description of Operations/Special Conditions	The description of operations entered and marked to print on the certificate holder.

CSV Columns	Description
Customer - Firm Name	The customer firm name and DBA, if entered for the commercial customer.
Customer - Last name, First name	The personal customer last name, first name.
Certificate Number	The number assigned by AMS360 or entered when the certificate was issued (created).
Certificate Description	The description entered for the certificate.
Policy Number(s)	The Policy Numbers for which the Certificatewas issued.
Cert Holder Name 1	The holder's name from the first line of the Namefield.
Cert Holder Name 2	The holder's name from the second line of the Namefield.
Cert Holder Contact Name	The contact name entered for the holder.
Cert Holder Addr 1	The first line of the holder's address.
Cert Holder Addr 2	The second line of the holder's address.
Cert Holder City	The holder's city.
Cert Holder State	The holder's state.
Cert Holder Zip	The holder's zip code.
Cert Holder Fax Phone	The holder's fax number.
Cert Holder E-Mail	The holder's email address.
Cert Issue Date	The date the certificate was issued.
# of Days notice	The number of days a cancellation notice was entered for the holder.
Certificate Holder Description	The description entered for the holder.
