Individual Address Change

Use the Individual Address Change service to submit one or multiple change of address, telephone, or email address requests to an individual licensee's resident license state and/or to multiple non-resident license states. Requests are processed with participating states through the NIPR Gateway or directly in "Sircon States" databases.

Note that there may be state and transaction processing fees associated with the use of this service.

The process involves the following steps:

Getting Started

  1. In the AgencyEDGE navigation bar, click the Transactions link. The Transaction List will open in the Home page.
  2. In the State Service Request group, click the Address Change Request - Individual link to proceed.

Enter Basic Information

Enter basic identifying information about the licensee. The system will validate the information against state data.

  1. Required. In the Last Name field, enter the licensee's last name. Maximum 35 alphabetic characters.
  2. Required. In the Social Security Number field, enter the licensee's Social Security Number. Maximum 9 numeric characters. Value will be auto-formatted.
  3. Required. In the Date of Birth field, enter the licensee's birth date. Maximum 10 characters. Use the mm-dd-yyyy date format; value will be auto-formatted. Or, click the Calendar button () to open a popup calendar from which you can click to select a date. Date entered must be 18 years prior to the system date.
  4. Required. From the Resident State dropdown menu, select the licensee's resident license state.
  5. Required. In the Resident License Number field, enter the number of one of the licensee's resident state licenses. Maximum 15 alphanumeric characters.
  6. Click the Next button to proceed.

Enter Licensee and Submitter Information

The page header displays confirmation of the licensee's Name, SSN, Resident State, and resident state License Number after validation against state data.

The Individual Info section will display if the licensee's resident state requires the licensee's telephone or email address information.

In the Submitter Info section, you must specify whether you are submitting the contact information change request on your own behalf or on behalf of the licensee. If you are submitting on behalf of the licensee, you are required to enter submitter information.

  1. In the Phone field, enter the licensee's business phone number. Maximum 10 numeric characters. Value will be auto-formatted.
  2. In the Email field, enter the licensee's business email address. Maximum 72 alphanumeric characters.
  3. From the Are you the licensee? dropdown menu, select Yes if you are the licensee submitting the contact information change request on your own behalf. Proceed to step 14

OR

Select No if you are submitting the contact information change request on behalf of a licensee. Proceed to step 4.

  1. Required. In the First Name field, enter the submitter's first name. Maximum 25 alphabetic characters.
  2. Required. In the Last Name field, enter the submitter's last name. Maximum 25 alphabetic characters.
  3. Required. In the Firm Name field, enter the name of the submitter'scompany. Maximum 100 alphanumeric characters.
  4. Required. In the Phone field, enter the submitter's company's telephone number, including area code. Maximum 10 numeric characters. Value will be auto-formatted.
  5. Required. In the Email field, enter the submitter's or your department's email address. Maximum 72 alphanumeric characters.
  6. Required. In the Address Line One field, enter the first line of the submitter's company's address. Maximum 50 characters.
  7. In the Address Line Two field, enter the second line of the submitter's company's address. Maximum 50 characters.
  8. Required. In the City field, enter the city of the submitter's company's address. Maximum 35 alphabetic characters.
  9. Required. From the State dropdown menu, select the state of the submitter's company's address.
  10. Required. In the Zip field, enter the postal or ZIP code of the submitter's company's address. Maximum 10 numeric characters. Value will be auto-formatted.
  11. Click the Next button to proceed.

Review Current Contact Information

The page header displays confirmation of the licensee's Name, SSN, Resident State, and resident state License Number after validation against state data.

The Update Contact Information section contains a simplified view of information for residence, business, and mailing contact information types in the states in which the selected licensee holds active resident and non-resident licenses. This information is obtained from state databases

  1. Review the current contact information recorded with each state in which the licensee is licensed.
  2. If desired, click the State column header to re-sort the list in reverse alphabetic order.
  3. In the Effective Date field, enter the date on which the contact information change(s) are to be effective. Maximum 10 characters. Use the mm-dd-yyyy date formats. Value will be auto-formatted. Date cannot be in the future, or, in some states, in the past.

  4. Click to select an address for which to change associated contact information. A popup window will open with edit fields allowing you to update the contact information for the selected type in the selected state or all contact information types in all license states.

Change Contact Information

If you selected to change the licensee's resident state contact information, you may apply the change to all contact information types in all license states. However, if you selected to change contact information in a non-resident license state, you may update only the selected contact information type in the selected state.

Review the Contact Change Request

The Summary Information section displays changed contact information for all selected contact information types in all selected states, together with the requested Effective Date.

The system has validated all changed addresses against valid address data obtained from the U.S. Postal Service. It checks to make sure that the requested changed address is an actual address. Changed address information that has been validated is ready to be submitted as a request to the selected state(s).

The Total Sircon Fee, Total State Fee, and Grand Total figures for the transactions display at the bottom right corner of the Summary Information section.

  1. Review the displayed information for accuracy.
  2. Review the fee totals with each requested change.
  3. To remove a change from the request, in the Action column click the Delete button associated with a selected change

OR

If all changes look good, proceed to step 4.

  1. Click the Next button to proceed.

Submit the Contact Change Request

In the Attestation section, you must swear that the information you are submitting is complete, truthful, and accurate. If you do, you are ready to submit the contact information change request(s).

  1. Read the attestation carefully.
  2. Required. When you are finished, if you submit to the terms of the attestation, click to checkmark the I Agree checkbox.
  3. Click the Next button. The Fee Summary page will open.
  4. On the Fee Summary page, review the details of the transaction, including applicable state, processing, and handling fees.
  5. If you need to change any data you have entered, click the Back button. Otherwise, if you are ready to proceed with the transaction, click the Continue button.
  6. When you are ready to submit your transaction together with your payment information, click the Submit button. The name change information will be transmitted to the state and update state records.

Complete the Process

The page header displays confirmation of the licensee's Name, SSN, Resident State, and resident state License Number.

You can review confirmation of your request in the Contact Change Submission Results section.

  1. Review the current status of each address change request in the Status column:
    • Approved: The state has accepted and processed the contact information change request, effective on the requested Effective Date.
    • State Review: The state is reviewing the contact information change request, and the request currently is in pending status. You may use Address Change Status to follow-up on the request.
    • Denied: The request encountered an error at the state. Review the Comment for a description of the error, and resubmit or contact the state department of insurance.
  1. Note and record the value of the Confirmation No. associated with a pending request. It may be helpful in following up on a pending request using Address Change Status.
  2. Review the Submission Date and Total Fee information at the bottom right of the page.
  3. When you are finished, click the Transactions link in the navigation bar or breadcrumbs to open the Transaction List page

Once opened, the Individual Address Change service will display the names of states or territories in which the service is unavailable. To change contact information in one of these jurisdictions, you must submit contact information change requests through its website. Click the State Information link at the bottom of the page to find state contact information.

States available in the Resident State dropdown menu are limited to only those that are available through the Individual Address Change service. If the licensee's resident state is not available, you must submit contact information change requests through the state's website. Click the State Information link at the bottom of the page to find state contact information.

A contact information change request sent to a licensee's non-resident license state may require that the information matches that of the licensee's resident license state. A request to change a licensee's non-resident state license contact information that differs from that of the licensee's resident license state may result in an error.

Depending on state requirements, addresses for certain address types that contain a P.O. box may not be allowed.

A state may restrict the number of days prior to the submission date to make a contact information change effective. If you encounter an error based on the value entered in the Effective Date field, contact the state insurance department.

Some states do not allow a resident or non-resident license state contact information change when doing so conflicts with resident or non-resident state license status. If you encounter problems, please contact the state department of insurance.

If you have recently submitted a contact information change to your resident license state, please allow 5 to 7 business days for processing before submitting a new license application.

You may use Address Change Status to follow up with an address change request that displays State Review in the Status column on the Contact Change Submission Results section.
